S.T.R.I.V.E. in Prophestown Illinois S.T.R.I.V.E. provides rehabilitative treatment and training in self-care skills in a comfortable, homelike environment. The primary goal of the facility is to optimize resident levels of functional independence through the provision of rehabilitation treatment and training in self-care skills, thus assisting the individual to live as independently as possible. S.T.R.I.V.E. offers a wide array of rehabilitation services to meet each residents' individual needs. We are located in the Northwest Illinois community of Prophetstown, a picturesque town on the banks of the Rock River.

Admission Procedures

S.T.R.I.V.E. has developed basic guidelines for the determination of eligibility and continued placement. We invite you to print out a Preliminary Application for Admission. Prior to a determination of eligibility for placement, a completed application and any necessary supporting documents must be submitted for a diagnostic evaluation to be performed. The final decision of the admissions review team is contingent on the ability of the existing programs and services to adequately meet the identified needs of the prospective resident.

Upon receipt of the completed preliminary application and medical records, a personal interview should be scheduled with the prospective admittee and family or appropriate significant others. The purpose of the interview is not only to aid in gathering information for admission purposes, but also to afford the patient the opportunity to tour the facility and ask questions. Special consideration will be given to the individual's goals and expectations and to the suitability of available services.

To avoid delay, please do not send the Authorization for Release of Information form back to us at STRIVE. After you have completed the Medical Record Release forms, please send them to the appropriate locale, and they then will send us the medical records that you have requested.

Recreational Therapy

The Recreational Therapy Department utilizes recreation services to improve or maintain physical, mental, emotional, and social functioning and to assist residents in expressing independent lifestyles.

A diversity of creative, psychosocial, religious, physical, intellectual, and community-based recreational programs are provided seven days per week, including evenings and weekends.

Various community outings are regularly scheduled, including shopping, dining out, cultural and sporting events. This allows residents real-life opportunities to practice the financial management skills and social skills they have learned through their comprehensive rehabilitation programs. Also emphasized is community awareness training, including wheelchair safety, self defense, community resource utilization, and environmental accessibility.

Library Services

S.T.R.I.V.E. has access to the Prophetstown public library. It contains a variety of books on many subjects and residents may request materials through the Therapeutic Recreation office. The resident is responsible to see that materials are returned. A current list of books is available from the local library. Arrangements can be made to check out materials.

Computer Lab

The Computer Lab is located in the Therapy Annex, which houses computers with internet access for your vocational development and leisure enjoyment. The Computer Lab is open every day during the hours posted. When entertaining family and friends, you must accompany them in this room. No smoking, drinking, or eating is allowed in the Computer Lab. Board games and puzzles are available as well. To obtain these items, please contact the Certified Therapeutic Recreation Specialist.

Social Services

The Social Services Department assists residents and families in their initial transition to the facility and also assists residents in achieving and maintaining maximum levels of social and emotional functioning.

The Social Services Department plays an important role in assisting residents and family members in resolving social and/or psychological problems by providing one on one counseling, psychosocial group therapy, and support group meetings as well as providing appropriate referrals to other qualified professionals, discharge planning and follow-up services.

Through psychosocial groups, residents are facilitated in self-esteem building, increasing assertiveness skills, learning problem solving techniques, and social skills training through role play and discussion.

Developmental Training

All residents at S.T.R.I.V.E. participate in the Winning Wheels Developmental Training Program at least five hours per day, five days per week. The program is located nearby at the Lyndon Progress Center.

The Winning Wheels Developmental Training Program enables residents to acquire adaptive living skills by providing programming in areas such as independent living, nutritional planning, consumer education, and interpersonal skills training.

Residents also learn functional skills which can be utilized in their everyday lives. These include money management/personal budgeting skills, meal preparation and serving etiquette, awareness of current events, and leisure education/planning. The fully equipped gymnasium is utilized for psychomotor activities, athletic events and FUN!

Food Preparation: Preparation of food served at mealtime. Using repetition of cutting movements, peeling, dicing, etc. to strengthen muscles and increase functional independence.

Life Skills/Health: A group discussion designed to inform about medical advances and developing a healthy lifestyle.

Current Events: In-depth discussion about social issues, referencing newspaper articles and stories from the world wide web.

Keep Striving: This group focuses on the development of social skills through role play, peer interactions, and life experiences of others who have disabilities and the challenges they face.

Medication Class: Designed to help you understand your medications dosages, how they interact with other medications, and how to utilize adaptive equipment to self-administer medications.

DT Rap: A time for all residents to discuss concerns or ask questions regarding facility policies, Day Treatment, advocacy, or resident rights.

Vocational Training

The Vocational Training Program augments interdisciplinary rehabilitative treatment provided at S.T.R.I.V.E. and adds a vital link in the continuum of care. The program provides its participants with the training necessary to overcome physical, psychological, social and economic barriers, to facilitate their placement in community-based employment. residents use specialized adaptive equipment that enables them to operate computer hardware and software.

Physical and Occupational Therapy

Each resident is evaluated by licensed physical and occupational therapists. residents are assessed in the areas of muscle tone, movement, balance, endurance, ambulation, wheelchair maneuverability, strength, posture, and motor control. Gait and mobility training and passive and/or active assistive range of motion are provided to prevent and/or reduce contractures in both the upper and lower extremities.

Speech and Language Pathology

Residents are evaluated by a Speech and Language Pathologist and/or audiologist as necessary. Therapy is provided for residents with speech, language, voice, swallowing, or cognitive-communicative impairments. Evaluation and training with augmentative communication devices is also available.

Medical Services

Physician Services: Physician services are provided by General Medicine. The doctor has office hours on-site at our facility monthly. This allows us to have an even greater emphasis on increased prevention and early intervention. All physician visits are based on medical necessity and patient needs. Dr. Anthony Kwan, physical medicine specialist, also assists with evaluation of residents and makes recommendations for treatment.

Specialized Rehabilitative Services: If specialized rehabilitative services such as physical therapy, speech-language pathology, and occupational therapy are required by your comprehensive plan of care, the facility will make arrangements to provide these services. These specialized rehabilitative services must be provided by qualified personnel under the written order of your physician.

Dental Services: Dental services are available through Milestone Dental Clinic, including annual check-ups and prophylaxis. Restorative work is provided, based on pre-approval by Public Aid. If you do not have funds available for extensive dental care (i.e. restorative work), the Social Worker will explore alternative means of having care provided.

Optical Services: Annual vision screenings are provided and arrangements will be made for you to receive the services of an optometrist or opthamologist upon request. However, if you do not have funds available for needed services, your social worker will explore alternative means of having that care provided. The facility does not assume financial obligation for this service.

Pharmacy Services: The facility will provide pharmaceutical services (including procedures that assure the accurate acquiring, receiving, dispensing, and administering of all drugs and biologicals).

Podiatry Services: Arrangements will be made for podiatric care upon request. You may choose to go to the podiatrist of your choice. However, if you do not have funds available for needed podiatric care, your designated social worker will explore alternative means of having care provided. The facility does not assume financial obligation for this service.

Psychiatry/Psychiatric Services: Upon written order of your physician, you may be referred to mental health services provided by a psychiatrist, psychologist, or clinical social worker. However, if you do not have funds available for needed services, your designated social worker will explore alternative means of having care provided. The facility does not assume financial obligation for this service.
